OHSAA Adopts a new digital physical form

OHSAA initiates the use of PrivIT e-PPE to improve the health and safety of student-athletes.

Columbus, OH—The Ohio High School Athletic Association (OHSAA) has announced a partnership with PrivIT, a Columbus, Ohio, based company, to streamline the process for the management and collection of physical forms required for participation in high school sports. The OHSAA will be replacing the paper physical forms with the PrivIT e-PPE (electronic Pre-Participation Evaluation) secure online solution beginning May 1 for the 2014-2015 academic year.

Migrating the paper PPE process to the PrivIT e-PPE solution gives parents the flexibility to keep their child’s health information current, and since the information is only entered once, updating the information in the future is as simple as clicking a few buttons. PrivIT e-PPE will help high schools better facilitate the process of collecting personal health history information from athletes and save time and costs associated with managing the paper process.

Within PrivIT e-PPE, parents of student-athletes will complete the medical questionnaire that will prepopulate several forms required for sport participation. Upon completion, these forms will be printed and taken to the student-athlete’s physical exam. Parents will enjoy using PrivIT e-PPE to manage their student-athlete’s health history information, because in subsequent years, they will only have to update the information in PrivIT e-PPE as opposed to completing all the paper-based forms again.

PrivIT e-PPE is a patented and secure HIPAA compliant solution that includes a more comprehensive health history questionnaire that is easy to use and helps mitigate health risks related to sports. The reports generated by PrivIT e-PPE provide a detailed summary of an athlete’s health history and readily identifies health risk areas to support the physical exam. Taking the paper-based process and transitioning it online will provide athletic trainers and coaches more time to focus on the nearly 350,000 Ohio student-athletes.

With the support of our Joint Advisory Committee on Sports Medicine, we are excited to provide our high school members the PrivIT e-PPE solution as a service designed to enhance the sport pre-participation process,” said Dr. Dan Ross, Commissioner of the Ohio High School Athletic Association. “When you think about sports and your student-athlete, you think of only the best. You want your child to be active, learn discipline, be healthy and have fun. It’s our job to help our school districts provide a safe sporting environment."

OHSAA will begin rolling out PrivIT e-PPE throughout the State high school athletic departments starting May 2014. Future plans include onboarding over 200,000 seventh and eighth grade junior high school athletes, officials, and coaches.


Fall Sports and Physicals


Starting with Fall Sports we will be launching a new online registration system that will also include a new athletic webpage that will be linked to the Greeneview Schools website.  Students and families will need to create a username and password in order to get into the registration system and sign up for a sport.  You will view and read forms and sign off on the necessary paperwork and turn in your updated physical form and you will be ready to tryout.  More details will be coming in the next couple of months. 


Due to new guidelines from the national level we will not be having a physical night here at the school as we have in years past. Please schedule your child’s physical with your family doctor so you can avoid any delays in participation or tryouts.  Students must have a physical on file in the Athletic Office before they may start practicing or tryouts.

Congratulations, Logan Lacure on your State Wrestling Championship!

Logan is Greeneview's first ever State Wrestling Champion.

Congratulations Lady Rams Basketball team on their District Championship!

They will play in the Regional Finals this Saturday at Springfield HS - 1:30pm. Contact the High School for tickets.  


State Wrestling


Logan Lacure became the first Greeneview Wrestler to win State.  Congratulations to Logan and to all the wrestling Coaches that helped make this happen.  Coach Sizemore, Coach Lacure, and Coach Puckett. 


Armani Robinson lost 3-2 in the Finals to finish as State Runner Up for the second time in his career.  He has finished 2nd twice and 3rd one in his HS Career. Congratulations Armani for a great HS Wrestling Career.  Armani is undecided on his college choice but hopefully he will have opportunity to wrestle in college.

State Gymnastics  Olivia Bradds finished 24th in the State Gymnastics Meet on the vault with a score of 8.9 Congratulations Olivia for making State.  Hoping you can return next season. 

District Girls Basketball  The Girls Basketball team won Districts beating Hamilton Badin 55-40.  They advance to the Regionals (Sweet 16) to play Bishop Ready on Wednesday at 6:15pm at Springfield High School.  Tickets will be on Sale at the high school during regular school hours. All tickets are $6 purchased in advance here and $8 at Springfield.  Please buy your tickets here at the school.  We get to a percentage of sales if tickets are bought from us.  Go Rams !

Remember each athlete must have a physical on file in the Athletic Office before they can begin trying out for a team.  We do not want anyone to miss tryout days because this might affect their ability to be chosen for the team.

No physical – no practice or tryout.




No Smoking on any school grounds including at Athletic Events.
