Please complete the back to school form prior to the first day of school. 

 

At the beginning of each school year, Greeneview Local Schools will require parents/guardians to complete online Student Demographic and EMA (emergency medical authorization) forms utilizing the OneView system. These online forms replace the paper forms that were distributed in prior school years. The information contained in the online forms helps ensure that we have the most up to date information pertaining to your child. In addition, should any information pertaining to your child change throughout the school year, we ask that you update the information using the OneView system.

 

  • Please make sure you read the "Yes"/"No" questions carefully. Many are defaulted to "Yes" so please read thoroughly before you click submit. 
  • The Free & Reduced Lunch Application is completed through OneView. The link is on the right side of the page. 
  • The "Immunization Waiver" form is not required or not common. Please do not submit this form unless you have not vaccinated your children and are requesting one of the allowable exemptions under Ohio code.

Q. Forgot your username or password? 

A. Parents should have received an email with your account username to be able to reset your password using the "forgot password" link on the Oneview Parent Portal page.
