Children, who have turned five years old by September 30, and wish to attend kindergarten this year.  Your child DOES NOT need to be present for registration. We will be registering 2022-23 kindergarten students 2 days only, Tuesday, April 12, and  Wednesday, April 13, 2021, at the board office, 4 South Charleston Rd. Jamestown. Enrollment process will be through OneView Forms (we will have computer stations on site to complete the online enrollment). 


Parents who would prefer to complete the online submission portion prior to coming to the Board Office may due so following the steps below (optional).

1. Click here (OneView) to begin the online enrollment process by creating a parent account in OneView (If you already have a OneView account with Greeneview Local Schools, please log in to that account).  After creating an account and logging in, please select the "New Student Registration" option (even if your student shows up in OneView that is currently attending preschool, please select "New Student Registration".)

2. Make sure to select "Enrollment is for 2022-23."

3. You are able to upload the required documentation during the new student enrollment form, but you still need to bring those required documents to registration day.

4. Transportation Opt-In form is located in Oneview at the top after you complete the New Student Registration. Complete this if you need transportation for your new kindergartener.

5. Please make sure to still bring your child’s required documentation on April 12th and 13th from 1pm-7pm at 4 South Charleston Rd. Jamestown Ohio 45335 (see below)

Required Documents during Enrollment

  • Original/Certified Birth Certificate, Passport or Birth Record (no other documents are acceptable)

  • Custody documents if applicable (appointing custody, residential parent, etc.)

  • Adoption paperwork (Birth certificate listing adoptive parents or a copy of adoption decree)

  • Immunization Records

  • Parent/Guardian must provide government-issued I.D.

  • Proof of Residence:

    • AES (DP&L), (CenterPoint Energy) Vectren, Waste and Recycling service or Water bill in its entirety (must include name) the bill cannot be more than 60 days old – No other bills are acceptable

    • If a utility bill is unavailable:

      • A new rental/lease agreement signed by the parent/guardian and the person or company renting the property can be used for initial registration (lease in effect for less than 45 days); however, a utility bill must be provided to the student’s school within 30 days of enrollment.

      • Closing Documents including property address and signed by both the parent/guardian and the seller can be used for initial registration (if less than 45 days); however, a utility bill must be provided to the student's school within 30 days of enrollment.

    • A Residence Affidavit (Must be on our form) is required if the student meets any of the following criteria:

      • Student is 18 years of age or older and is living with someone, other than parent/guardian, within the district.

      • Student (less than 18 years of age) and parent/guardian are living with a family member or friend who resides within the district.

Physician Health and Physical

Kindergarten screening will be held May 26 and 27 at the Elementary School.  This will require approximately two hours for you and your child.  This is only an assessment screening to provide you with more information about the readiness of your child.  The screening includes hearing, vision, and various other basic evaluations.  This is not designed to determine acceptance, but as a service to the parents.  Parent decision and the child’s age are the determining agents in kindergarten entrance.  You will receive screening information at the registration.  This screening is important and appropriate even for children who may have been screened last year.
